<P>PROBLEM TO BE SOLVED: To provide a valve assembly and diameter reduction structurefor trocar using a movable diameter reduction structure. Ž<P>SOLUTION: The diameter reduction structure is arranged in proximity to a first seal and includes two or more standoff members which can move among a first location, a second location, and a third location. The diameter reduction structure in the first location reduces the possibility that the first seal loses its integrity by restricting the excessive off-axis and angular movements of small diameter surgical instruments. When an operating device with a large diameter is arranged through the trocar, this diameter reduction structure circles round so as to suit the passage of the operating device with the large diameter without any operation accommodation.
