PROBLEM TO BE SOLVED: To provide a mat foundation form allowing form plates of a corner part to be easily attached and detached in a short time and to be reused. SOLUTION: The mat foundation form is composed of support fittings 10 each comprising a horizontal support plate 5 for supporting the form plate of the corner part to a marking plate 3 near the corner part, an outer form holder 6 for supporting an outer form plate 11 provided at one end of the horizontal support plate 5, a vertical support plate 8 provided at the other end of the horizontal support plate 5 turnably through a hinge part 7, and an inner form holder 9 for supporting an inner form plate 15 provided at the tip part of the vertical support plate 8; an outer form 13 formed by allowing the end of one outer form plate 11 and a side face of the end of the other outer form plate 11 to abut on each other to form the corner part and fixing the corner part to the outer form holders 6 of the support fittings 10 with adhesive tapes 12; and an inner form 16 formed by allowing the end of one inner form plate 15 and a side face of the end of the other inner form plate 15 to abut on each other to form the corner part, and fixing the corner part to inner form holders 9 of the support fittings 10 with the adhesive tapes 12.
