An attachment for a machine tool is provided to restrict rotation of an external rotary shaft to rotate a main spindle attachment in the vertical plane when the main spindle attachment switches from a revolving state to a swiveling state. An attachment(21) for a machine tool is detachably attached to a lower surface of a ram(11). The attachment includes a fixed table part(22) to be fixed to the lower surface of the ram, a horizontal swivel part(23) supported slidably and rotatably relative to an inner surface of the fixed table part, and a vertical swivel part(24) supported slidably and rotatably relative to a side surface of the horizontal swivel part. The fixed table part is provided with a clamp shaft(31) for fixing the fixed table part to the ram. The clamp shaft is designed to be clamped or unclamped by a fixed-table-part clamping apparatus provided in the ram. By unclamping the clamp shaft, the attachment is caused to descend away so that the attachment is detached from the ram.
