PROBLEM TO BE SOLVED: To efficiently and high-accurately perform collation confirmation operation of a mouthpiece and prevent erroneous operation and failure of a radio ID tag due to heat. SOLUTION: An extrusion head 3 is equipped with a head body 4 fixed to a front end of a rubber extrusion body 2 and a pre-former set 6 having the mouthpiece 14. The mouthpiece 14 is equipped with a mouthpiece identification means 22 composed of a plate-shaped and metallic mouthpiece body 21 forming an extrusion port 7 and the radio ID tag 24 storing the information X of the mouthpiece, and a tag holder 25 for holding the radio ID tag 24 and mounted to the side surface of the mouthpiece body 21. The tag holder 25 is made of a synthetic resin material and the radio ID tag 24 is non-contactingly fixed to the mouthpiece body 21. A reading sensor 26 which reads the information X of the mouthpiece stored in the radio ID tag 24 by approaching and leaving the radio ID tag 24 is mounted to the head body 4.
