<P>PROBLEM TO BE SOLVED: To form an RZ (Return to Zero) optical signal, an intensity multi-valued optical signal and a phase multi-valued optical signal in a simple configuration and in a miniaturized shape. <P>SOLUTION: A semiconductor substrate 31 with which two optical modulators 11 and 12 are integrated in series is mounted on a semiconductor substrate 32 with which driving circuits 21 and 22 are integrated.
