An LTCC(Low Temperature Co-fired Ceramic) tape composition, an LED module, a lighting device and a method for forming the same are provided to be easily applied to a variety of lighting devices. At least two LTCC tape layers are supplied. One or more cavities are formed on each LTCC tape layer. Two or more electrical vias and one or more thermal vias(504,506) are provided to the LTCC tape layer. The LTCC tape layer is capable of supplying an circuit pattern. A functional chip carrier is obtained from the resultant structure by connecting electrically the circuit pattern with the electrical vias.
