PROBLEM TO BE SOLVED: To provide an integral-type form which can adapt to printing by various types of printers, and which enables reduction in printing cost and an increase in working efficiency by integrating various forms such as an application postcard with a concealing label, a payment handling slip, a delivery slip and article specifications. SOLUTION: Two or more types of forms are provided in a unit slip; a transparent base material 104, an adhesive layer 105 and a release member 106 overlie a part of the backside of a printing base material 101 which is commonly used for at least a part of the two or more types of forms; and a thickness adjusting base material 108, which is as thick as the base material 104, the adhesive layer 105 and the release member 106 and which eliminates a step, is provided in an area except an area which consists of a laminate of the base material 104, the adhesive layer 105 and the release member 106 together.
