PROBLEM TO BE SOLVED: To provide a form milling tool capable of increasing a re-grinding frequency, by maintaining a form as the whole tool, by allowing a tool tip position after re-grinding and the form of an outer peripheral blade to follow each other by the re-grinding of only a rake face of the outer peripheral blade. SOLUTION: This form milling tool has a bottom blade and the outer peripheral blade. The form milling tool is characterized in that a shape of a flank of the outer peripheral blade is formed in the shape of gradually shifting a phase of the form of the outer peripheral blade in the blade root direction of the outer peripheral blade backward in the tool rotational direction, and when an angle in the direction for shifting the phase to a line perpendicular to the tool axis in a tool side view is set toαand an angle of a clearance angle of the bottom blade is set toβ, an expression ofβ- 0.4°≤α≤β+ 1.2°is satisfied, and when re-grinding the rake face of the outer peripheral blade, a retreat quantity of the tool tip position of a form milling and a dislocation quantity of the form of the outer peripheral blade are allowed to follow each other.
