PROBLEM TO BE SOLVED: To provide a flexible printed board less influenced by a wiring load. SOLUTION: A first face 41 located on the uppermost layer of the flexible printed board 30 is formed into a substantially L shape or substantially T shape composed of a long part 42 and a short part 43. A second face 44 is formed from a side part of the short part 43 of the first face 41 extending in a direction substantially perpendicular to a longitudinal direction of the long part 42 along the longitudinal direction of the long part 42, and is folded by substantially 180°by a folded line forming a boundary with the short part 43 and is positioned as a second hierarchy below the short part 43. A power supply connector 31 is mounted on the long part 42 of the first face 41, and a laser diode driver IC32 equipped with a recorded waveform pattern generation circuit is mounted on the short part 43 of the first face 41.
