PROBLEM TO BE SOLVED: To provide a form paper for making a booklet with an index which uses no metal fitting that has a danger of piercing a finger or others. SOLUTION: A first face part 5 and a second face part 6 are provided on the opposite sides of a central face part 2 with folding lines 3 and 4 interposed and the upper edge 5a of the first face part 5 is cut off by an index width M demarcated beforehand from the upper edge of the central face part 2, while the lower edge 5b thereof is extended by the double of the index width M from the lower edge 2b of the central face part 2 and a cutting line 7 is provided along a position located at a half width of the extension. As for the second face part 6, the upper edge 6a thereof is cut off by the double of the index width M from the upper edge 2a of the central face part 2, while the lower edge 6b is extended by the double of the index width M from the lower edge 2b of the central face part 2.
