PURPOSE: A form for a concrete structure is provided to execute an installation work for a form economically and effectively. CONSTITUTION: The form for a concrete structure contains a support(S), a yoke bar and a joist(10) installed together with the support(S), a slab from installed on the upper part of the joist(10), and a tunnel form(W) forming a wall form, wherein many joist supports is installed on the inner top of the tunnel form at the appointed intervals, and the joist support contains a joist placer provided with a depressed part to place/house the joist(10) and a wing projected outward from both sides of the joist placer.
