PROBLEM TO BE SOLVED: To provide an optical semiconductor device module the optical axis of which is aligned with good reproducibility. SOLUTION: An optical fiber collimator 22 having a 2nd lens 3 and an optical fiber 4 mounted is connected to the external liner 18 of a positioning liner 17 which reaches a mount 6 by penetrating from the outside to the inside of a package 8, and a first lens is arranged in the internal liners 19a and 19b. Since the internal liner 19 is fitted into an annular guide groove 20 on the mount 6 while adopting the optical axis B of the optical semiconductor device 1 mounted on the mount 6 as the center, the central axis C of the positioning liner 17 is easily aligned with the optical axis B with good reproducibility.
