PROBLEM TO BE SOLVED: To provide a method for forming a planar optical waveguide which is simple, speedy, and inexpensive, to also provide the inexpensive planar optical waveguide, and to provide a method for forming the planar optical waveguide which easily provides the function as a passive optical element to the planar optical waveguide and the planar optical waveguide which is provided with the function as the passive optical element. SOLUTION: The optical waveguide is formed by pressing a molding die having the shape of the desired optical waveguide on a moldable flat material having a layer to be a lower clad and a layer to be a core of the optical waveguide. In the method for forming the optical waveguide, the optical waveguide is formed by pressing the molding die having the shape of the desired optical waveguide on the moldable flat material having the layer to be the lower clad and the layer to be the core of the optical waveguide.
