PROBLEM TO BE SOLVED: To provide a communication terminal device which can easily identify a form where an image of a communication management document such as an error report is formed. SOLUTION: Facsimile equipment is equipped with the communication terminal device which includes a recording part 3 for forming an image on a form, a read part 2 for reading the image, a modem 9 and a NCU 10 for receiving the image, and a control part 1 for controlling the recording part 3 to form various images including a management report on a 1st surface and a 2nd surface of the form and the supply of the form to the recording part 3 and discharges the form with the 1st surface down; and a message is stored in a ROM 6 and the control part 1 controls the recording part 3 to form the message image on the 2nd surface of the form where the management report is formed and the supply of the sheet to the recording part 3.
