The document is made with a number of sections (13, 14) and includes a detachable card made from an end flap with folded and glued sections (181, 182, 185) giving at least two thicknesses. At least one surface of the resulting card is laminated with film, while another is glued to an adjacent surface. The card is connected to the remainder of the document by a perforated line or other detachable connection, and it can incorporate a row of unique identification characters.
