PROBLEM TO BE SOLVED: To provide improved tool for applying a clamping fastener to a spinal implant and an anchor assembly without cross-threading. SOLUTION: A cartridge 10 installs the clamping fastener such as a set screw 3 and a lock nut 5, for example, to fix a linking rod 72 or cable in the head of the anchor assembly. The cartridge carries one or more fasteners or pairs and applies them onto the anchor head without cross-threading. An alignment guide 20 aligns and guides the cartridge in the state of fitting the cartridge to a centering body 24 with jaw mechanism. One cartridge has a box wrench, which externally grips and turns the nut, and a central shaft coaxial with the box wrench, which positions and turns a clamp screw in cooperation with the nut. Preferably, an inner lock screw 3 is attached coaxially with an outer nut 5 but in front of it by a spring or the like and before pressing, extending and final fastening of the spinal column, a linking member is fixed on the anchor assembly.
