PROBLEM TO BE SOLVED: To provide a form plate investigated from a form removing surface as a countermeasure, by which a cost for executing an earthquake-resistant slit is reduced, and an execution method using it. SOLUTION: In forms 1, a joint molding member 2 is installed on the concrete-placing surface side 4 of the form plate 3, the joint molding member 2 is mounted by fixing by nails 6 to the form plate 6 and a batten cleat 5, an alkali-soluble release sheet 8 formed of a woven fabric or a nonwoven fabric is stuck previously on the exposed surface 7 of the joint molding member 2. In forms 10 and 20, molding members 11 and 21 for the earthquake-resistant slits are secured near the end sections of the form plates at regular intervals, and the alkali-soluble release sheets 8 are stuck.
