PROBLEM TO BE SOLVED: To provide an information recording medium which is excellent in reliability by suppressing the position-deviation of a header between adjacent tracks to the minimum extent without lowering the whole storage capacity. SOLUTION: In the information recording medium of the shape of a disk having a track formed in a spiral shape, one or more header fields are disposed to each circumference of the track. The interval between the header fields continuing in the track direction is adjusted by the integral multiple unit of a prescribed length Lw so that the deviation in the track direction of a n-th header field in a circumference different from a prescribed circumference is minimized to a header field which is a reference for the n-th circumference in the prescribed circumference of the track.
