PROBLEM TO BE SOLVED: To provide a delivery slip having a simple handling of a warranty and improving an operating efficiency. SOLUTION: The delivery slip 1 comprises a slip base 2, a pressure sensitive adhesive 5 coating on a rear surface side of the base 2 via an auxiliary base 4, an attached slip 9 and delivery slip 8 covered with a release sheet 6, a warranty seal 11 continuously connected to a lower end of the slip 9, a warranty label 27 partitioned by a slit 26 arriving at the surface of the sheet 6 from the base 2 so that the label 27 is separable, while a slit 29 provided at a position corresponding to an outside of the slit 26 for partitioning the label 27 on the sheet 6, and a label protective part 6a.
