PROBLEM TO BE SOLVED: To realize a form device for reducing cost and labor for repairing the form device by preventing an appearance fault of a product due to an invasion of a fluid such as a concrete slag or the like to a superposed part of a design mat attached to the device. SOLUTION: The form device (1) comprises the design mat (14) mounted in a form for forming a pattern on a surface of a member (12) to be molded. The device (1) also comprises blocking members (38), (40) provided to bridge over between an end face of the mat (14) and a surface of the form connected to the end face of the mat (14) to prevent an invasion of the concrete slag to the superposed part of the form and the mat (14).
