The document consists of a standard-format paper backing layer (1) coated with an adhesive (2) on one side, and an inner siliconized layer (3) attached to the backing layer by the adhesive. The document has a series of fold lines for making it into a pouch, and the siliconized layer has a number of pre-perforated lines (9) forming tear-off strips (10) that leave adhesive strips when removed for sticking the edges of the pouch together and forming a stick-down flap. The completed pouch has an additional flap along one edge for information, and can have a removable patch for an address label.
