PROBLEM TO BE SOLVED: To allow easy cut off at the time of use and to enhance the fabricatability of slips at the time of their production. SOLUTION: The slip 1 with labels has a basic piece 10 of #A and divided pieces 11 of #B to #L. The slip 1 with the labels is successively joined with the basic piece 10 of #A and the divided pieces 11 of #B to #L via the labels stuck thereto on the rear surface. The slip has perforations 13 between the divided pieces 11 of #B to #L. The basic piece 10 of #A has the half-cut perforations 12 cut over the entire surface in the portions for successively joining the divided pieces 11 of #F to #L on the rear surface and has the cuttable perforations 13 in the portions where the labels 20 are not stuck. The basic piece 10 of #A is provided with corner cuts 14 between the half-cut perforations 12 and the perforations 13 for successively joining the divided pieces 11 of #L, by which the failure at the time of disconnection by the offset of the perforations is prevented.
