PROBLEM TO BE SOLVED: To provide an inexpensive form for FAX-OCR, which does not stain a hand at handling and, in addition, is suitable for realizing easy execution of a printing work and has a favorable handleability. SOLUTION: A form A1 for FAX-OCR is produced by binding together a FAX-OCR paper for FAX-OCR and a copying paper 2 locating on the lower position side of the paper 1. The copying paper 2 is made of a self-copying paper 3. Thus, when a predetermined information is entered in the information entry column on the surface 1a of the FAX-OCR paper, the information is copied on A self-copying surface 3a, which is the surface of the self-copying paper 3, and on the rear surface 1b of the FAX-OCR paper 1 contacting with the self-copying surface, resulting in allowing to eliminate the conventional copying of the information to the rear surface 1b of the FAX-OCR paper with the spot carbon.
