PROBLEM TO BE SOLVED: To provide an optical device mounting base and an optical module having a positioning marker having a good detection precision in which a loading groove for an optical waveguide body such as optical fiber or optical waveguide can be precisely positioned to the positioning marker for an optical element. SOLUTION: A base 1 has a loading groove 1a for loading an optical waveguide body 11 which is formed by etching, an electrode pattern 5 for loading optical elements 12, 13 to be optically coupled to the optical waveguide body 11, and a plurality of opening markers 10 consisting of etching resisting films resistant to the etchant used for the formation of the loading groove 1a, which are formed with a prescribed distance from the loading groove 1a.
