Business order form with gummed edges - has pocket to receive cheque to be sent in payment

A business order form is made from one large sheet which is folded along the edge (17) and sent to the customer. The customer removes part of the upper sheet by tearing along the line of perforations (19) after having filled in his order. The remaining portion (12) of the upper sheet forms a pocket of suitable size to receive the cheque used for payment. The form is then folded along the central line (10) and sealed by moistening the gummed edges (11). The dimensions of the form are approx. 20 cm x 30 cm.
