Interventional medical closure device

A medical device for creating and closing an opening through a vessel wall is described. The device includes a guiding engagement element positioned in a vessel wall; an incising member positionable over the engagement element; and a cutting element, with a recessed notch, at a distal end of the incising member. The cutting element forms a flap on a first portion of a vessel wall, the flap having a first portion attached to the wall and second portion having a first diameter cut and a second diameter cut larger than the first diameter cut. The engagement element includes a retaining element engaging a portion of the flap; and an anchoring element engaging an external surface of the vessel, wherein the retaining element and the anchoring element exert a compressive force on the vessel wall for assisting closure of the cut.

1. A medical device comprising:
a sheath member for inserting an engagement element through a vessel wall into a vessel lumen, the engagement element comprising an expandable retaining element on a distal end, the retaining element adapted to engage a flap in the vessel wall created by an incision to the vessel lumen to move the flap to seal across an opening through the incision; an incising member positionable about the sheath member, the incising member comprising a cutting element located at a distal end of the incising member, the cutting element being adjustable between an open configuration and a closed configuration, wherein the cutting element forms the opening when in the closed configuration to accommodate the sheath member therethrough, thereby enabling the cutting of the vessel lumen upon adjustment of the cutting element to the closed configuration without cutting the engagement element; wherein the cutting element is dimensioned to cut into a portion of a wall of the vessel lumen to form a flap in the wall, the flap having a portion attached to the wall; and wherein the engagement element comprises: a retaining element configured to engage a portion of the flap; and an anchoring element configured to engage a portion of the wall, the retaining element and the anchoring element configured to exert a compressive force on the flap and on the wall until the incision is sealed.
