PROBLEM TO BE SOLVED: To provide a tamper-resistant transdermal opioid delivery device which includes opioid or a pharmaceutical acceptable salt thereof, and an acyl opioid antagonist or a pharmaceutical acceptable salt thereof. SOLUTION: The transdermal delivery device includes: an analgesically effective amount of the opioid or the pharmaceutical acceptable salt thereof; and, when the transdermal delivery device is used as indicated and if a detectable amount does not permeate human's skin, and after the transdermal delivery device is misused and the absorption into human's blood flow has occurred, the acyl opioid antagonist or the pharmaceutical acceptable salt thereof which exists in the transdermal delivery device in an amount enough to control an inebriety effect of the opioid or the pharmaceutical acceptable salt thereof.
