PROBLEM TO BE SOLVED: To provide an image forming apparatus and an image forming method capable of embedding additional information to an output image in a form whereby a user can hardly recognize the embedded additional information. SOLUTION: A signal generator 52 discriminates whether or not a signal is modulated for a corresponding scanning period according to a bit signal of an embedded data sequence 502. That is, when the embedded data bit 50x is 0, no modulation is carried out for a period x corresponding thereto. As a result, an image signal is expressed in a signal 504 as shown in Figure. When the embedded signal bit 50y is 1, the modulation is executed to bring the frequency of the original image signal into an optional different frequency for a period y corresponding thereto. As a result, the image signal is modulated as shown in a signal 506 in Figure.
