PROBLEM TO BE SOLVED: To improve a sliding property of a sleeve part to a supporting shaft. SOLUTION: This apparatus is provided with a base 11 in which a supporting shaft 13 is provided in parallel to an optical axis of an objective lens 3, a lens holder 12 having a sleeve part 16 in which the supporting shaft 13 is inserted through, holding the objective lens 3 at one end side, and being able to slide in the direction of tracking and focusing, magnets 15a, 15b, a drive part having first to fourth tracking coils 18a-18d for rotating a focusing coil 17 and the lens holder 12 in the direction of tracking for driving the lens holder 12 arranged in the magnetic field generated by this magnets 15a, 15b in the direction of focusing. a flexible wiring substrate 19 which is arranged from a bottom plane of the base 11 to an upper side curvedly, of which the end part is attached to the lens holder 12, and which supplies a current to first and second coils, a neutral member 24 which is arranged at the sleeve part 16 obliquely and generates rotation moment being reverse to rotation moment applied to the lens holder 12 by the flexible wiring substrate 19 by being attracted by the magnets 15a, 15b.
