PROBLEM TO BE SOLVED: To provide service activity recording slips of a support expense system capable of easily and surely preparing and entering a unifying service code in the service activity recording slips used for an office work processing in the support expense system. SOLUTION: The service activity recording slips 2 record contents of service activities of service providing persons in the support expense system. Service activity recording copy slips 3 on which the same printing as those of service activity recording slips 2 are made, are provided and the contents entered on the recording slips 2 for the service providing persons can be duplicated thereon. On both slips, at least serial slip numbers 5, items for specifying the service providing persons and users, dates for service activities, and the contents of the service activities are printed. Furthermore, on the service activity recording slips, service code entry columns 14 consisting of code numbers of regal sections of the support expense system, code numbers of kinds of the services and code numbers of the service contents, and a table 15 for codes on the service contents are printed.
