PROBLEM TO BE SOLVED: To provide a delivery slip, whose slip for pasting can be separated whereby a corrugated carton or the like can be re-utilized but very hard to be peeled naturally upon delivery and whose delivery slip can easily be taken out, further, the treatment of whose delivery slip can be effected systematically and whose slip for pasting can be utilized as a customer's slip. SOLUTION: A third slip A for pasting in the delivery slip consisting of five sheets is provided with a main slip 3 for pasting, whose bound side of the lower surface and the outer periphery of three sides surrounding the same are applied with a re- separatable adhesive agent 11, and an auxiliary pasting piece 4, superposed on the other side of lower surface of the main pasting slip 3 and bonded, while the re- separatable adhesive agent 11 is bonded to the outer periphery of four sides. A separatable layer 12 is applied on the outer periphery of four sides of a customer's copy slip 6 for separating of the fifth sheet. The fourth delivery slip 5 is provided with the same size as the inner diameter of the re-separatable adhesive agent 11 around the outer periphery of three sides of the main pasting slip 3 and a turning piece 5a at the other end is superposed on the end of the main pasting slip 3 while the rear surface of the auxiliary pasting piece 4 is bonded to the substantially whole surface of the upper surface of the same.
