PROBLEM TO BE SOLVED: To miniaturize a body operation part into a compact one and to precisely operate an ultrasonically operated wire by disposing a first and a second rotary member fitted inside the body operation part and comprising an operation means of the operation wire in front/rear positions in different surfaces of a support plate. SOLUTION: The inside space is vertically divided into two by the support plate 44 fitted in a casing 40 of the body operation part 2 and extended axially of the body operation part 2. A side opposed to a cover 43 of the support plate 44 is formed wider than a space in the side where an angle operation means 25 is installed. A first pulley assembly 22 is provided in the front surface side of the support plate 44 and a second pulley assembly 24, which is larger than the first pulley assembly 22, is installed in the rear surface side. An electric motor 23 is also installed in the rear surface side of the support plate 44 and an operation shaft 37 and an input shaft 26a penetrate through the support plate 44 and are extended to the rear surface side.
