PROBLEM TO BE SOLVED: To provide a form for molding a culvert improving maintainability. SOLUTION: A core (3) is arranged in an outer frame (2). The core (3) has a lower core structure (31) arranged on an upper part of a bottom plate (6) of the frame (2), and an upper part core structure (30) slidably arranged on an upper part of the structure (31). The structure (30) has right and left side plates (33, 33) at right and left side edges of a top plate (32) and a side plate enlarging and contracting means (34) for enlarging and contracting between the lower end edges of the right and left side plates (33, 33). In this case, the lower end edges of the right and left side plates (33, 33) are constituted so as to be closely contacted with the right and left end edges of the structure (31).
