PROBLEM TO BE SOLVED: To provide a form support device suitable for use at the time when concrete is placed in a ceiling part. SOLUTION: A jack support 1 is provided with a base 101 at one end in the axial direction and the other end is a free end 102. One end side of a first cylindrical body 2 is incorporated with the free end 102 of the jack support 1 to be made transferable along the axial direction of the jack support 1. A second cylindrical body 3 is fitted to the first cylindrical body 2 and transferred along the axial direction of the first cylindrical body 2. A first adjusting mechanism 41 is provided between the jack support 1 and the first cylindrical body 2 to adjust the height of the other end of the first cylindrical body 2. A second adjusting mechanism is provided between the first cylindrical body 2 and the second cylindrical body 3 to adjust the relative position of the second cylindrical body 3, regarding the other end of the first cylindrical body 2 as the standard.
