<p>A sleeve (21) for a surgical instrument includes a hollow tube (20) having a passage sized to accommodate the surgical instrument, and a seal (75) disposed within the passage. Threads on an exterior surface of the tube engage the skin. An element (65) disposed at the proximal region of the tube is configured to engage a corresponding element of a tool to transmit torque from the tool to the sleeve, thereby to rotatable advance the sleeve into the skin. The threads increase in diameter in a proximal direction. The tool element non-locking engages the sleeve. A device for inserting the sleeve includes a post (185) configured to receive the sleeve.
