A pattern formed sheet, a forming mold and apparatus for manufacturing the sheet are provided to extend storage period of an optical sheet on which a pattern is formed. An apparatus for manufacturing an optical sheet comprises a first roll around which a base film is wound; a second roll around which an optical sheet is wound; and a master roll on which a forming mold(142) is carried, formed between the first roll and the second roll. The forming mold comprises a stitch connection unit(200) connecting at least one or more forming molds and a guide protrusion(400) arranged toward a set specific direction with respect to a stitch connection unit.
