<P>PROBLEM TO BE SOLVED: To provide a prepaid card-attached form integral type card mount easy to handle in a convenience store. <P>SOLUTION: The prepaid card-attached form integral type card mount 1 includes a prepaid card 10, a mount 100 which separably holds the prepaid card 10, and a mount body side hanging hole 111 and a coating side hanging hole 131. The mount 100 includes a mount body part 110 with a card arrangement part 112 and a form part 120. The form part 120 has a form side bar code 121 in which electronically read information is recorded. The prepaid card 10 includes a front surface with a card side bar code 11 in which electronically read information is recorded. The prepaid card 10 is held by the mount 100 so that a rear surface is arranged between the front surface and the card arrangement part 112.
