A device for opening and closing a vehicle door has a rail for a slide door, the rail being fixed to an outside part of a vehicle compartment and extending in the front-rear direction of the vehicle; a support member for supporting the slide door and moving along the rail; a first pulley and a second pulley that are supported at longitudinally opposite ends, respectively, of the rail; a cable turned back at the first and second pulleys and placed in an annular shape by being engaged to the support member; and a drive unit for rotatably supporting a drive wheel and fixed to an inside part of the vehicle compartment, the drive wheel having the cable wound around the drive wheel to reciprocate the support member according to the direction of rotation of the drive wheel. Further, the drive unit is supported by the rail so as to be able to rock between a first attitude at which the drive unit projects from that surface of the rail at which the rail is mounted to the vehicle body and a second attitude at which the drive unit lies along the surface of the rail.
