PROBLEM TO BE SOLVED: To provide an optical transmission substrate preventing signal light from diffusing when emitted from a light penetrating hole, a method of manufacturing the optical transmission substrate, an optical transmission apparatus, a compound optical transmission board, and a photoelectric hybrid board. SOLUTION: The optical transmission substrate comprises: a substrate 1 provided with a through-hole 5 between two main faces; a first refractive index body 3a having a refractive index which is larger in radial direction than in circumferential part in the through-hole 5 and provided along the center axis of the through-hole 5 from the opening part on one main face side of the through-hole 5 to the inside of the through-hole 5; a condenser lens 3c inscribed in the opening part on the other main face side of the through-hole 5, separated from the first refractive index body 3a and having the same optical axis as that of the first refractive index body 3a; and a second refractive index body 3b having a refractive index which is smaller than those of the first refractive index body 3a and the condenser lens 3c and provided in the through-hole 5 at the periphery of the first refractive index body 3a and the condenser lens 3c.
