PROBLEM TO BE SOLVED: To provide a delivery slip which needs no release paper to temporarily stick to a pressure-sensitive adhesive layer, contributes to resource saving and environmental preservation, facilitates the removal of its delivery slip 9 and a receiving slip 10 from a delivered article, and enables the cutting of the delivery slip 9 on which personal information is displayed by a shredder. SOLUTION: The delivery slip is characterized in that paying a special attention on adopting a heat-activating pressure-sensitive adhesive in a pressure-sensitive adhesive region 3, the surface of a slip substrate 2 is coated with the heat-activating pressure-sensitive adhesive to prepare the pressure-sensitive adhesive region 3, and a region other than the pressure-sensitive adhesive region 3 is made to be a printing region 4, the printing regions 4 are arranged between a first pressure-sensitive adhesive region 3A and a second pressure-sensitive adhesive region 3B of the pressure-sensitive adhesive region 3 respectively through region boundary parts 5 and 6, the first pressure-sensitive adhesive region 3A and the second pressure-sensitive adhesive region 3B are made bendable to the printing regions 4 at the region boundary parts 5 and 6, and the printing regions 4 can be cut from the first pressure-sensitive adhesive region 3A and the second pressure-sensitive adhesive region 3B at the region boundary parts 5 and 6.
