Disclosed is a percutaneously absorbable ophthalmic preparation, which can retain a therapeutically effective concentration of a heterocyclic spiro compound or a salt thereof that acts to increase the tear secretion and which produces less adverse side effects such as miosis. A percutaneously absorbable ophthalmic preparation comprising a heterocyclic spiro compound represented by the general formula (I) or a pharmaceutically acceptable salt thereof as an active ingredient: wherein each symbol is as defined in the description.
