A patient (30) concretion (44) destruction ratio (Z) determination procedure takes transmission images (40, 54, 62) during shock wave treatment and automatically extracts concretion correlated statistical values (M, S) from which the destruction ratio is derived from an automatically recognised zone. Independent claims are included for a shock wave treatment system using the procedure. Shock wave treatment ends automatically when the destruction ratio reaches a preset value (G).
