A form assembly assembled with an insert and a spacer, and an installation method are provided to make a spacer member assembled with a form, to make an insert member assembled with the form, and to make the form assembly supported by a general pipe support. A form assembly assembled with an insert and a spacer comprises plural through combining parts each comprising a through hole member pierced in a form(1100); form-combining spacer parts(200) each comprising a spacer part combined with the through combining part; form-combining insert parts(700) each comprising an insert member combined with the through combining part; a filling bolt part each comprising a bolt member combined with the through combining part; and auxiliary supporting parts each comprising a plate-shaped member fitted between the form and a general pipe support. The through combining part includes a filling nut part(110) comprising a nut with a female thread formed along the inside of the through hole member. The filling bolt part fills up the through combining parts if the form-combining part or the form-combining insert part is not used. The auxiliary supporting part enables the general support pipe to support the form by receiving a bolt head of the form-combining insert part or the filing bolt part protruded to the lower part of the form.
