<P>PROBLEM TO BE SOLVED: To provide an apparatus for creating a mail form, which makes a workpiece pressure-bonded without being damaged, in terms of the mail form to which the workpiece is annexed. <P>SOLUTION: This apparatus for creating the pressure-bonding mail form folds a continuous body of the pressure-bonding mail form to which the workpiece is annexed and which is coated with a pressure-sensitive adhesive, cuts off each unit form, and pressure-bonds it by passing it between a pair of rolls. Characteristically, the apparatus has a sealing device 30 which is composed of a pressure roller 301 which has a recessed part 301S of a size equivalent to the size of the annexed workpiece, so that the annexed workpiece can be passed between the paired rolls without being damaged; and a pressure roller 302 which faces the pressure roller 301 in parallel with it.
