PROBLEM TO BE SOLVED: To provide an index card which can be filled out with index information (recording order and recording time), related to the recording information to be recorded to a disk with fine appearance even by hand writing. SOLUTION: The index card 10, used by being housed in a disk housing case 80, is provided with matrix-like substrates 14 and 15, in such a manner that characters, symbols, marks, etc., can be respectively easily written in number display columns (tracks) 13a to be filled in with the recording order and time display columns (time) 13c to be filled in with the recording time.
