A steel mold for formation of underground box structure is provided to stably construct with mortar for forming ceiling of underground box structure. A steel mold for forming underground box structure comprises: a step of installing a rail(20) inside the underground box structure in the longitudinal direction; a step of putting a wheel(220, 320), which is installed on the bottom of a main support frame(200) and auxiliary support frame(300), on the rail; a step of hinge-combining a jack bolt(610) on the side of the main support frame and auxiliary support frame at a regular interval; fixing a cylinder(500) and a jack bolt(400) on the upper side of the main support frame and auxiliary support frame; a step of hinge-combining the cylinder and a jack bolt(410) on the side of the main support frame and auxiliary support frame; a step of combining a horizontal steel form(110) upward with the cylinder and jack bolt which are fixed on the upper side of the main support frame and auxiliary support frame; a step of combining an arch steel form(120) with the cylinder and jack bolt which are fixed on the upper side of the main support frame and auxiliary support frame; and a step of combining the horizontal steel form and arch steel form using a hinge(121).
