PROBLEM TO BE SOLVED: To prevent pressure on the management of a hospital or a clinic due to the sharp increase of labor costs and the waiting state of an outpatient for a long time due to the shortage of help by sharply rationalizing a reception task such as the revisit and reservation of the outpatient at the window of the congesting hospital or the clinic or the like. SOLUTION: This reception device is provided with a rewrite reader/writer 1 rewritable many times, outside equipment 3 such as an automatic record retriever, and on-line computer 2. Then, a revisiting patient sets a rewrite card type consultation ticket at the rewrite reader/writer 1, selects a consultation subject, and inputs the consultation subject and the consultation content at that date from an inputting device such as a screen touch or a keyboard to a computer 2.
