<P>PROBLEM TO BE SOLVED: To provide an order sheet, a preparation method for it and a utilization technology for it which are full of flexibility. <P>SOLUTION: A plurality of mark areas CKB arranged at optional positions on an order sheet, a content explaining portion printed at a position corresponding to each mark area and a sheet content information code CD1 are printed on the order sheet PRS.
