<p>PROBLEM TO BE SOLVED: To provide a slip with a card which can be supplied at a low price and to which a ready-made card can be attached. SOLUTION: This slip with a card is structured of a slip paper 2 and a card 5 applied to the surface of the former leaving a column for printing information intact. The card 5 is releasable by attaching the card 5 with a releasable layer 9 formed on the back to the surface of the slip paper 2 through an adhesive layer 12. Compared to a conventional-type slip with a card, this slip with a card uses no expensive false adhesive material for a sheet, so that it can be supplied at a low price. Consequently, the total cost can be cut when a large quantity of the slip with a card is supplied.</p> |
