PROBLEM TO BE SOLVED: To enable address information to be read even with high recording density by providing a plurality of recording sectors each including a header region which includes a plurality of address regions each being provided with a postamble region at the end thereof and non-pit data or a space disposed at the end of the postamble region. SOLUTION: A track 1b is formed on an optical disk 1a in a spiral-manner and the track 1b is divided into recording sectors 1c in accordance with a predetermined physical format. The recording sectors 1c are arranged successively in the circumferential direction to form the track 1b. At the head of each of the recording sectors 1c, a header region 2 is provided so as to previously record therein addressing information for reading address information. The header region includes a plurality of address regions and each of the address regions is provided at the end thereof with a postamble region. Data in the address region is modulated, based on the state.
